RECURRING DEFECTS · THE REPETITION GAP

Your defect keeps coming back. Years apart. Nobody has asked why.

If your team has stopped asking that question, this page is written for the people who still have to answer for the repeated outcome.

30 years in automotive tooling. Governing programmes since 2003.

tooling and defect inspection

This is what shows up on the surface

Not a category. The three we’ve actually seen, in the customer’s own words. Hover to look closer.

A case we’ve seen. Not you, but you’ll recognise it.

One real supplier, held anonymously. If this sounds familiar, that’s the point.

01

Report delivered

DFM + Moldflow review.

02

Silence, then the same complaint

Sink marks. Flash. Poor moulding.

03

Years later: same defects

Same words. Nothing had changed.

You already run this process

The question was never whether these stages exist. It’s whether each one was done in full depth.

STAGE 1

Engineering Design

Already visible here, to anyone looking

A-019

DFM Sign-Off

Passed. But how deep, really?

A-020

Moldflow Sign-Off

Passed too. On the real material?

A-022

Steel GO

No more chances after this.

Point of no return

Everything before this point looked fine. That is exactly the problem.

Without governance
  • ×A sink mark is visible at the design review. It gets noted, not stopped: we’ll catch it later.
  • ×DFM sign-off happens, but for the sake of happening, not in full depth.
  • ×Moldflow runs, but not necessarily on the real material or the real wall-thickness map.
  • ×Steel is cut. The defect is now permanent, until the tool is corrected or re-cut.
With Hongyi JIG
  • ✓Every gate has exit conditions. The next stage cannot start until the current one is signed off complete.
  • ✓DFM and Moldflow are verified in full depth, not assumed complete because a report exists.
  • ✓A benchmark sample, where available, is checked against the design before steel is committed.
  • ✓Steel GO happens only once, with the risk already closed, not carried forward into the tool.

The answer is not the one most people give

Most of the time, we already know the reason.

Knowing does not change anything.

The same mistake keeps happening. To all of us.

We all know this. Nobody enforces it.

Ask any of us, 25 or 30 years in, why the defect came back, and you’ll hear one of these four. Every one of them is true. Not one of them is the reason.

Everyone was stretched thin

01

Knowledge sits with one person. Authority to stop sits with another.

02

What worked before quietly rewrites the rule

03

Knowledge lives in a person, not in the project

04

More experience usually means less attention per project

Every answer above is easy to find. None of them is where the problem actually happened.

Where did we actually lose the key? (Watch this story to understand it better)

Time. Luck. One engineer. Bandwidth. Four answers, all standing in the light, because that is where it is easiest to look. The real answer is sitting somewhere darker, back at a stage nobody thought to check twice. The founder tells that story himself.

Full effort. Sincere search. Wrong spot. Not a people problem. A governance problem.

So what’s actually missing

Not more knowledge. Discipline and order, the kind that comes through governance.

What this looks like inside your business

Five patterns. If two or more feel familiar, keep reading.

THE PAIN

Same defect. Every programme.

Flash, cut off by hand. Every time.

WHAT GOVERNANCE DOES

Traced to origin, not cut off by hand

One owner. Root cause. Closed at the source.

THE PAIN

Tool-making vendor reports: limited independent verification

Verification of vendor technical inputs is often limited.

WHAT GOVERNANCE DOES

Verified before commitment

Checked before you’re committed, not after.

THE PAIN

Supplier decisions driven mainly by price and lead time

Technical fit is often not assessed deeply enough before award.

WHAT GOVERNANCE DOES

Qualified before the trial

Not found unfit after it fails.

THE PAIN

You are the only fix point

You can’t be everywhere.

WHAT GOVERNANCE DOES

Control that scales without you

Structure holds, even where you can’t.

THE PAIN

Design changes cascade

You already know which deadline you’ll miss.

WHAT GOVERNANCE DOES

Validated before freeze

Caught before freeze, not after.

What we actually do here

Not your engineering team’s replacement. The party that traces the failure and holds the record.

01

We do not replace your engineering team

02

We verify DFM and Moldflow before steel, not after

03

Reference sample verified before tooling proceeds

04

You keep ownership. We give you the evidence trail

01

Which product or programme keeps showing the repeat defect

02

Roughly how many programmes have seen the same pattern

03

Whether your current supplier has been told about the repeat, and what they said

Where do you want to go next
